At Columbine Health, the support doesn’t end after move-in day. Social workers play a crucial role in helping residents and their families adjust, understand their benefits, and feel heard every step of the way.
From insurance coverage and financial concerns to emotional wellbeing and community resources, social workers are both practical problem-solvers and compassionate advocates.
“One of my biggest responsibilities is educating families,” said Kylie Thompson, Social Services Director at Lemay Avenue Health and Rehab. “But I’m also an advocate, a listener, and a stable presence during a very uncertain time.”
Thompson shared the story of a resident worried about returning home alone. “He didn’t feel safe managing meals,” she said. “After one conversation and a referral to Meals on Wheels, he was relieved and excited to go home. Sometimes small solutions make a big impact.”
Social workers also collaborate closely with care teams and external partners to make sure transitions are smooth and residents stay supported. Brunner recalled a particularly complex case involving multiple insurance denials. “It took creativity, advocacy, and teamwork,” she said. “But we stayed committed to her success and found a path forward.”
For Von Dreele, empowerment starts with listening. “We want residents to feel in control of their journey. It starts by asking what matters most to them.”
