Person-Centered Care

We feel it is our honor and privilege to provide health care and living environments for older adults in Northern Colorado. Since 1971, our residents, staff, and families value our mission and vision. We believe they reflect who we are and why we dedicate ourselves to caring for our community each and every day. We hope you will look through our Guide to Columbine if you are seeking care for you or a loved one and can find the service or facility we offer to meet your needs. If you would like to come on board as an employee, we welcome you too!

Living That Fits Your Lifestyle

PATIO HOMES -- Our Patio Homes are built for active adults, 55 and older who desire a home of their own without the responsibility of maintaining it. That’s right, you are not responsible for the property taxes, property insurance, or any worries about the maintenance, landscaping, lawn care, or snow removal. With a full time dedicated maintenance person, our patio homes allow you to focus on the activities you want to do, not what you have to do.
INDEPENDENT LIVING -- Our independent living facilities are designed for seniors who are able to live on their own, but desire the security and conveniences of community living. We offer a lifestyle complete with recreational, educational, and social activities with other seniors.
ASSISTED LIVING -- Assisted living at Columbine Health Systems is a balanced lifestyle. New Mercer Commons in Fort Collins, Lakeview Commons in Loveland, and Columbine Commons in Windsor strive to maintain resident independence without sacrificing their care. Residents are encouraged to remain active and independent while receiving assistance with those daily living activities that have become difficult.
SKILLED NURSING -- Columbine Health Systems boasts five skilled nursing facilities, including Columbine West Health & Rehab Facility,Lemay Avenue Health & Rehab Facility, North Shore Health & Rehab Facility, Centre Avenue Health & Rehab Facility, and Columbine Commons Health & Rehab Facility Services include 24 hour skilled nursing care, physical, occupational, respiratory, and speech-language therapy, secure memory units, group and individual activities, and transportation to medical appointments
